Amy S. Kim, DDS, is a clinical associate professor at UW’s School of Dentistry and assistant director of clinical research at UW’s Center for Pediatric Dentistry. She has worked in private practice and in the SMILE Program, which provides in-school dental care. Experienced in dental academia with a focus on admissions and interprofessional education, Kim’s research interests include project/program management, regulatory affairs and human subjects protection.
Elise Sarvas, DDS, MSD, MPH, is a board-certified pediatric dentist and clinical assistant professor in the Division of Pediatric Dentistry at the UMN School of Dentistry.
Kathy J. Eklund, RDH, MHP, is the director of occupational health and safety at the Forsyth Institute in Cambridge, Massachusetts. She is also an adjunct faculty member at the Dental Hygiene Program at Regis College, School of Health Sciences in Weston, Massachusetts. Eklund is a member of the

John Y. Kwan, DDS, is an associate clinical professor at the University of California, San Francisco School of Dentistry. A diplomate of the American Board of Periodontology, he maintains a periodontal practice in California’s Oakland/Berkeley area, and also serves as clinical director for Perioscopy, a division of Zest Anchors, LLC.
Kenneth L. Reed, DMD, is a dentist anesthesiologist and a diplomate of the American Dental Board of Anesthesiology and National Dental Board of Anesthesiology. He is the associate program director in dental
anesthesiology at New York University Lutheran Medical Center in
Brooklyn. He is also an affiliate assistant professor in the Department of
Periodontology at the School of Dentistry, Oregon Health & Science
University in Portland, and a clinical instructor in the Department of
Dentistry at the University of Alberta in Edmonton, Canada. Reed is president of the American Dental Society of Anesthesiology and maintains private practices limited to anesthesiology for dentistry in Arizona, California and Nevada.
Andrea M. Fonner, DDS, is a dentist anesthesiologist and a
diplomate of the American Dental Board of Anesthesiology and National
Dental Board of Anesthesiology. She serves as a basic life support
instructor for the American Heart Association, and is an associate faculty
member in the General Practice Residency at the Swedish Medical
Center in Seattle. Fonner is the codirector for the American Dental Society of Anesthesiology Human Simulation Course, and provides advanced medical emergency training to dental teams across the country. She is also the president and delegate for the Washington State component of the American Dental Society of Anesthesiology.
